As a Mental Health Therapist, you will play a key role in
delivering therapeutic services including conducting assessments,
guiding group therapy sessions, developing treatment and discharge
plans, and ensuring accurate patient care documentation. The ideal
candidate will have strong analytical skills, a friendly and
approachable demeanor, and the ability to work collaboratively in a
team environment.
Key Responsibilities:
--- Conduct comprehensive mental health assessments for
patients
--- Lead group therapy sessions with a focus on patient engagement
and support
--- Develop individualized treatment and discharge plans
--- Maintain accurate and up-to-date patient care documentation
--- Provide ongoing support to patients, adjusting care plans as
needed

Responsibilities:
--- Assess client needs and build customized solutions for behavior
issues while establishing individual targets and goals
--- Ensure that all relevant clinical information is documented
properly and sent to the patient, their referring healthcare
provider, and the treatment team
--- Perform regular wellness checks by phone to inquire about
behavioral health status and medication adherence
--- Develop treatment plans based on the patient's values, goals,
and health using comments from the patient's treatment team and
support system
--- Provide group and individual therapy to patients and families,
using evidence-based approaches
--- Assist with intervention for patients when a crisis occurs
--- Refer patients and family members to community resources or to
specialists as necessary
--- Evaluate patients' circumstances on a regular basis and provide
necessary follow-up care
--- Determine the status of the patient's mental health by using
medical information, interviews, observation, and testing, as well
as collaboration with other staff members
--- Develop specific therapy plans with goals and objectives for
individual patients
--- Create treatment plans that reflect patients' values, goals,
and health while incorporating feedback from treatment team
--- Access client needs and build customized solutions for behavior
issues while establishing individual targets and goals
--- Support patients and their families and their families with
evidence based therapy in both group and individual settings
Qualifications:
--- Current and unrestricted state license as an LCSW (licensed
clinical social worker) or equivalent
--- Must have a high school diploma or GED equivalent and a
bachelor's degree in counseling, social work, or related field
--- Exceptional problem-solving and analytical ability as well as
outstanding interpersonal and communication skills are needed
--- Requires CPR and BLS (Basic Life Support) certifications
--- 1-3 years in a clinical setting working with behavioral health
patients is required
--- Active LCSW (Licensed Clinical Social Worker) or LCPC (Licensed
Clinical Professional Counselor) license
--- SWLC (Social Worker Licensure) or PCLC (Professional Counselor
Licensure Candidate) candidates are welcome to apply
--- Strong interpersonal skills and ability to work with diverse
patient populations
--- Analytical mindset with attention to detail in treatment
planning and documentation
