Responsibilities:

--- Prepare patient documentation and communicate pertinent clinical information to the patient, referring provider, and treatment team
--- Conduct phone wellness checks on a regular basis to enquire about behavioral health and medication compliance from patients
--- Develop treatment plans based on the patient's values, goals, and health using comments from the patient's treatment team and support system
--- Evaluate client needs and create individual interventions while establishing person-centered goals for behavior modifications
--- Provide group and individual therapy to patients and families, using evidence-based approaches


Qualifications:

--- Additional certifications required: CPR, BLS (Basic Life Support)
--- State license that is current, unrestricted, and in good standing for a clinical social worker (LCSW) or equivalent
--- High school diploma or GED and a bachelor's degree in counseling, social work, or equivalent
--- Exceptional problem-solving and analytical ability as well as outstanding interpersonal and communication skills are needed
--- Active LCSW (Licensed Clinical Social Worker) or LCPC (Licensed Clinical Professional Counselor) license
--- SWLC (Social Worker Licensure) or PCLC (Professional Counselor Licensure Candidate) candidates are welcome to apply
--- Strong interpersonal skills and ability to work with diverse patient populations
--- Analytical mindset with attention to detail in treatment planning and documentation
